Indiana Hoosier Healthwise Children's Health Insurance Program
Hoosier Healthwise is for Indiana children under 19 years of age who meet the financial requirements. Applicants must be U.S. citizens or qualified aliens. Pregnant women can qualify for Hoosier Healthwise coverage as well.
Hoosier Healthwise covers:
- Doctor visits
- Routine check-ups
- Well-child visits
- In-patient hospital
- Prescription Drug Coverage
- Diagnostic Lab and X-rays
- Mental Health Care
- Dental and vision care
There are no remiums. Co-payments range from none to minimal for e.g. prescription drugs.
Membership in Hoosier Healthwise last for for 12 months. Members complete an eligibility review application to renew for additional 12-month terms.
