Iowa Hawk-i
Hawk-i coverage is for uninsured Iowa children under the age of 19 whose families' income is within the family income guidelines. Children must also be a U. S. citizen or qualified alien. There is no waiting period for the hawk-i program.
The hawk-i program provides the following coverage:
- Doctor Visits and Routine Check-Ups
- Prescription Drugs
- Hospital In-patient care
- Emergency care
- Diagnostic Labs and X-ray
- Dental Care
- Eye exams and Eyeglasses
Wellmark Classic Blue Health and Dental, Wellmark Health Plan of Iowa Blue Access and Blue Dental, John Deere Health Plan and Delta Dental provide coverage under the hawk-i program.
The premium that members pay varies between $10 to $20 a month. There is no co-payment or coinsurance for most services. A co-payment of $25 may be required for emergency care.
Members are covered for 12 months at a time. If eligibility requirements. are still met, members can renew of an additional term.
