Nevada Check Up Children's Health Insurance Program
Check Up is for uninsured Nevada residents under 19 years old. Eligible applicants will also be U.S. citizens or legal residents in the U.S. for 5 years. The family's income must also be within the family income guidelines. Applicants must have been without group health insurance for the last six months or currently be eligible for Medicaid. Certain children may be exempt from this waiting period.

- Well-Child Visits and Immunizations
- Dental and Vision Care
- Hospital Care
- Lab and Radiology
- Prescription Drugs
- Mental Health Care
Members will be assigned to an HMO. Nevada Medicaid must be used if HMO providers are not available in the member's area.
No co-payments, no deductibles, no co-insurance are required. Premiums are pegged to family income and family size.
Check Up membership is for 12 months. Renewal is offered prior to the end of the 12 month term.
