Utah Children's Health Insurance Program (CHIP)

Utah CHIP is for uninsured state residents 18 or younger. Applicants must also be American citizens or legal residents and their families must meet the posted family income guidelines.

Utah CHIP covers:

Doctor Visits
Well-Child Exams, Hearing and Eye Exams
Prescription Drugs
Hospital Care
Labs and X-rays
Mental Health
Preventive Dental
Physical Therapy

Utah CHIP is offered by Molina Health and Public Employees Health.

Premiums range from $0 to $25 per quarter. Small co-payments may be required for services. Total family out of pocket expenses will never exceed 5% of their annual income.

Coverage is for 12 months and requires a renewal application for an additional term.
