Coverage
- A PPO plan with deductibles ranging from $1,000 to $10,000.
- An indemnity plan with a $1,000 deductible.
- All plans have 20% coinsurance in-network and the PPO plans have a 40% out-of-network coinsurance. Out-of-pocket maximum varies from $2,500 to $15,000 depending on deductible selected.
Eligibility
- Alaska for 12 months minimum
- Not eligible for any small group health insurance
- Not eligible for federal or state health insurance
- Uninsured
- One of the following conditions must apply: 1) were declined for coverage in the last 6 months by another health insurance company, 2) were offered coverage with riders that exclude important benefits, or 3) have a pre-existing medical conditions stated in eligibility guidelines
