Health Insurance Plan Iowa (HIPIOWA)
Plans
- Plan A: For people under 65 who are on Medicare
- PPO Plan B: $1,000 deductible in-network and $2,000 out-of-network. $2,500 in-network and $5,000 out-of-network annual out-of-pocket maximum
- PPO Plan C: $1,500 deductible in-network and $3,000 out-of-network. $3,000 in-network and $6,000 out-of-network annual out-of-pocket maximum
- PPO Plan D: $2,500 deductible in-network and $5,000 out-of-network. $5,000 in-network and $10,000 out-of-network annual out-of-pocket maximum
- All plans: a 20% coinsurance in-network and 40% out-of-network.
Eligibility
- Iowa resident for 60 days minimum
- Must meet one of the following:
- Declined for health insurance within the last 9 months, with a benefit exclusion or limitation, or c) you were approved for a health insurance policy, offered coverage at a cost higher than HIPIOWA
- Applicant has a medical condition on the HIPIOWA presumptive list
- HIPAA eligible had group health insurance within the last 63 days and exhausted COBRA
