Plans
- Traditional Access: a fee-for-service plan with $400 deductible and 15% in-patient coinsurance or 20% outpatient coinsurance. Out-of-pocket max is $1,500 after deductible.
- Premier Access: a PPO plan with choice of deductibles: $700, $1,500 and $2,250 and 35% coinsurance in-patient and 40% for out-patient services, the. The maximum out-of-pocket limit is $2,500 for the $700 deductible plan, $3,000 for the $1,500 deductible plan and $4,500 for the $2,250 deductible plan.
- Preferred Access: a PPO plan with choice of two deductibles: $750 and $1,500 and 40% coinsurance. The out-of-pocket maximum for the $750 deductible is $2,500, and for the $1,500 deductible it's $5,000.
Eligibility
Applicants must have been Kentucky residents for12 months or more and also meet one of the following requirements.
- HIPAA eligible: Applicant was recently insured on a group health insurance policy, had 18 months of continuous coverage, and coverage terminated less than 63 days from application to Kentucky Access.
- Rejected for individual health insurance coverage, or offered coverage at a higher rate than a similar Kentucky Access plan.
- Applicant has one of the medical conditions listed in the Kentucky Access eligibility guidelines
- Applicant is in the state Guaranteed Acceptance Program
- Applicant is a spouse or child of a person on the Kentucky Access program
