Plans
- A and J: $1,000 deductible, 25% coinsurance, $3,500 annual out-pocket maximum after deductible.
- B and K: $2,000 deductible, 25% coinsurance, $4,500 annual out-pocket maximum after deductible.
- C and L: $3,500 deductible, 25% coinsurance, $4,500 annual out-pocket maximum after deductible.
- D and K: $5,000 deductible, 25% coinsurance, $4,500 annual out-pocket maximum after deductible.
Eligibility
- Uninsured Louisiana resident for 6 months or more
- Declined by two health insurance companies within 12 months of applying for LHP or insured by an individual health insurance plan that costs more than 200% of the LHP premium
- HIPAA eligible - Applicant recently had at least 18 months group health insurance in Louisiana.
