Plans
- PPO Plan with choices of $500, $1,000, $2,000, $5,000 and $10,000 deductible. After deductible is met, co-insurance is 20% in-network with $3,000 maximum out of pocket expense, except the $5,000 and $10,000 deductible plans in which the deductible is your out-of-pocket maximum.
- High Deductible Plan that qualifies for Health Savings Accounts (HSA). Coverage is 100% after deductible.
- Medicare Supplemental plan pays the 20% coinsurance after the Medicare deductibles are met.
Eligibility
- Uninsured resident of Minnesota for 6 months or more immediately prior to MCHA application date
- Denied health insurance by a Minnesota insurance company
- Over 65 and not eligible for Medicare
- Eligible for guaranteed issue coverage under HIPAA
- Eligible for Trade Adjustment Assistance or Pension Benefit Guaranty Corporation
- Treated for a 'presumptive' medical conditions in the MCHA eligibility guidelines
