Plans
- PPO Plan 1 - $500 deductible with $2,500 maximum out-of-pocket expense
- PPO Plan 2 - $1,000 deductible with $5,000 maximum out-of-pocket expense
- PPO Plan 3 - $2,500 deductible with $5,000 maximum out-of-pocket expense
- All plans have a 20% coinsurance in-network, 50% out-of-network
Eligibility
- Missouri resident
- Declined for individual health insurance in the last six months due to pre-existing condition, or
- Offered individual health insurance at a premium exceeding 300% of the standard rate for individual coverage, or
- Previously covered by a high risk pool in another state, or
- Exhausted COBRA coverage, or
- Prior coverage terminated beyond control of applicant
