Plans
- Traditional Plan - with choice of $1,000, $2,500, or $5,000 deductibles with 20% coinsurance. Annual out-of-pocket maximums are $5,000, $6,000, or $7,500
- HIPAA Plans - with choice of $1,000, $2,500, or $5,000 deductibles with 30% coinsurance. Annual out-of-pocket maximums are $5,000, $5,000, or $8,000
Eligibility for Traditional Plan
- Montana resident for at least 30 days
- Declined for private health coverage by two insurance companies within the last 6 months
- Offered private coverage with a restrictive rider by two insurance companies within the last 6 months
- Offered private coverage at a premium exceeding 150% of the MCHA rate
- Applicant has an qualifying illness listed by MCHA
Eligibility for HIPAA
- Uninsured Montana resident for at least 30 days
- Prior group health coverage for 18 continuous months terminated within 63 days of MCHA application
- Exhausted COBRA continuation coverage if available
