Plans
- PPO plan with a choice of 9 deductibles from $500 to $10,000. All have 20% coinsurance and out-of-pocket maximum of $1,500 after deductible
- Health Savings Account (HSA) qualified plan also available.
Standard Eligibility
- Nebraska resident for at least six months
- Medically declined for private health insurance coverage in the last six months
- Offered private health insurance with a restrictive rider limiting or excluding a pre-existing medical condition
- Offered private health insurance at a higher rate than comparable NCHA coverage
- Have a medical condition listed by NCHA
HIPPA Eligibility
- Uninsured Nebraska resident
- Covered for 18 continuous months on a group health plan
- Exhausted COBRA coverage and the premium for the premium for the continuation coverage is higher than comparable NCHA coverage
