Health Plan Options
- Indemnity plan with deductibles of $2,000 or $3,500 and20% coinsurance The out-of-pocket maximums are $5,000 and $7,000 respectively.
- Managed Care Plans with deductibles of $1,000, $2,500, and $5,000 and20% coinsurance for in-network services. Out-of pocket maximums are$3,500, $5,000, and$7,500 respectively.
- Managed Care Plan with $5,000 deductible and no coinsurance. Once deductible is met, covered expense are 100% paid.
Health Plan Eligibility Requirements
- New Hampshire resident
- Declined for individual health insurance for health reasons within the last 6 months, or
- Offered coverage at a higher premium than a similar NHHP plan
- Offered coverage with an exclusion rider for a specified condition
- Exhausted any COBRA coverage if available
- Eligible for federal trade act assistance or pension benefit guaranty corporation assistance
- Have one a medical conditions as specified by NHHP
