- Option 1 - $1,000 deductible with 20% coinsurance and $3,000 maximum out-of-pocket expense includes deductible
- Option 2 - $500 deductible with 20% coinsurance and $3,000 maximum out-of-pocket expense expense includes deductible
Standard Eligibility
- North Dakota resident for 183 days or more
- Denied health care insurance coverage for health reasons, or
- Offered coverage with substantially restrictive conditions, or
- Offered coverage at a rate exceeding the CHAND rate, or
- Diagnosed with a CHAND qualifying condition
- Ineligible for North Dakota's medical assistance program
HIPAA Eligibility
- Uninsured North Dakota resident
- Prior group health insurance coverage for a minimum of 18 months and
- Applied for CHAND within 63 days of the termination of prior coverage and
- Exhausted COBRA coverage is available
- Ineligible for coverage under Medicare or Medicaid
- Ineligible for coverage under the medical assistance program of North Dakota
TAARA Eligibility
- North Dakota resident
- Ineligible for Federal Trade Adjustment Assistance
- Prior insurance coverage of at least three months and
- Applied for CHAND within 63 days of the termination of prior coverage and
