Plans
- PPO Plans with a choice of $500 or $1,000 deductible with 20% coinsurance. The maximum out-of-pocket expense is $1,000.00 or $4,000 after deductible respectively.
- Medical and Portability Plan with a choice of $750 or $1,500 deductibles with 20% coinsurance. The maximum out-of-pocket expense is $3,000.00 or $6,000 after deductible respectively.
- coinsurance is 30%. The maximum out-of-pocket limit is $6,000.00 after deductible.
Medical Eligibility
- Was declined for private health insurance for health reasons, or
- Were offered individual health coverage with a waiver that substantially restricted or excluded coverage, or
- Diagnosed a medical condition listed on the OMIP application
Portability Eligibility
- Prior group health insurance coverage for at least 18 months and
- Exhausted COBRA coverage if offered and
- Applied for OMIP with 63 days of prior plan termination or
- Previous insurance company ceased operations in the State of Oregon
