Plans

• PPO Plan with choice of 1,000, $3,000 or $10,000 deductibles and 25% coinsurance after deductible is met.. The out-of-pocket maximum, including deductible, is $3,250, $5,250 or $12,250 respectively.
• Health Savings Account (HSA) with $3,000 deductible and 25% coinsurance. The maximum out-of-pocket  annual expense, including deductible, is $5,000.

Eligibility

• Uninsured South Dakota resident, and
• Prior group health coverage for at least 12 continuous months, and
• Exhausted COBRA or state continuation coverage if eligible
• Applied for SD Risk pool coverage within 63 days of termination of prior coverage, and

The program does not provide health insurance coverage to uninsured individuals that are unable to obtain coverage from private insures, unless they have also recently lost creditable coverage.
