Plans
- Plan 1 is a standard PPO plan offering deductibles of $500, $1,000.00 and $1,500. Coinsurance is 20%. The maximum out-of-pocket limit, including deductible, is $1,000 for the $500 deductible, $1,650 for the $1,000 deductible and $2,000 for the $1,500 deductible.
- Plan 2 is a Medicare supplemental for individuals participating in Medicare Part A and Part B. There is no annual deductible and the maximum out-of-pocket limit for plan 2 is $500 annually.
- Plan 3 is a PPO Plan with a choice of deductibles, including $500, $1,000, $2,500 and $5,000 deductibles, 20% coinsurance. The maximum out-of-pocket limit, including deductible, is $1,000 for the $500 deductible, $1,650 for the $1,000 deductible, $5,000 for the $2,500 deductible and $10,000 for the $5,000 deductible.
Eligibility
Must be a resident of the State of Washington, and one of the following:
- Been declined for coverage by another health insurance company in the past 90 days
- Do not have individual health insurance available from one of the association health insurance companies
- You are eligible for coverage under federal HIPAA regulation
- You must not be eligible for a group health plan or Medicare or Medicaid
